Apply for equipment

If you are a parent of a disabled child (or a therapist or personal assistant) you need to consider the following before applying:

• Is your child aged 18? (we need to receive your application before their 18th Birthday)
• Does your child have a permanent condition that affects their mobility?
• You have approached your local NHS wheelchair service and the mobility equipment your child needs is not available.

To receive a hard copy of the application form or assistance in completing the form, please call our Children’s Services on 0845 052 1231 (Local rate).
